Hm, interesting. Others Americans who have received honorary knighthoods include former New York City Mayor Rudolph Giuliani, former President George Bush, and Ronald Reagan.
 
However, Sir Elton John is also a Knight... Recognition from any organization is only as valuable as the merits of the least deserving recipient.

If the queen sends your knighthood invitation, can you refuse? I'm just curious.
 
The way it goes is: they quietly agree behind closed doors whom is deserving of honors. They then ask that person wether they are willing to accept such honors. If they say yes, they proceede with a public announement. If the answer is no, they just let it go without a nomination. They don't want to offer a medal and then be turned down and embarrassed.
